Eli Lilly Hits a Record High. Q3 Tests Whether Foundayo Is Real.

Mounjaro, Zepbound, and a new oral pill are growing at once. The question is whether Foundayo's ramp is fast enough near $1,273.

Eli Lilly briefly crossed $1,300 intraday on October 20, a fresh all-time high driven partly by the European Medicines Agency recommending approval of Foundayo, its oral obesity pill, on October 15. The stock has since pulled back to roughly $1,273. On October 29, the company reports third-quarter earnings, and the number that matters most is not Mounjaro or Zepbound, it’s Foundayo, and how fast doctors are actually prescribing it.

The Business

Lilly is the largest pharmaceutical company in the world by market cap. Mounjaro, its injectable tirzepatide for type 2 diabetes, generated about $8.7 billion in Q1 2026. Zepbound, the same molecule sold for obesity, has been climbing steadily. Together they helped drive Q1 revenue up 56% to $19.8 billion and Q2 revenue up 48% to $23.0 billion.

Foundayo is orforglipron: a small-molecule GLP-1 receptor agonist that patients can take at any time of day without food or water. The FDA cleared it on April 1, and Lilly said it began shipping in early April. It is the first oral GLP-1 for weight loss that does not require timing restrictions. The EMA’s positive opinion this month puts European commercial launch within reach.

Why Wall Street Is Paying Attention

Thirty-one analysts cover LLY, and the average price target is about $1,332. Goldman Sachs raised its target to $1,413 last week. Both carry Buy ratings. The bull case centers on oral GLP-1 penetration: RBC analyst Trung Huynh projects Foundayo could reach $36 billion in peak annual sales, which would surpass the current record held by Keytruda.

The complication is pricing. Novo Nordisk’s oral Wegovy launched with a $149-per-month self-pay offer for starting doses, forcing the market to recalibrate Foundayo’s revenue assumptions sharply downward. Buy-side forecasts for 2026 Foundayo sales fell after the price war became clear. Foundayo generated just $98 million in its first full quarter of availability. Early data showed most prescriptions were from patients new to the GLP-1 class, which is the best possible signal on market expansion, but the ramp is slower than the most optimistic models assumed.

What’s Driving the Opportunity

Consensus for Q3 sits near $22.5 billion. Phase 3 data for retatrutide, Lilly’s next-generation triple agonist, showed average weight loss of 28.3% of body weight over 80 weeks at the highest dose, a number that would reset the obesity treatment ceiling if it reaches approval. Lilly has also said eligible Medicare Part D individuals may be able to get Foundayo for $50 per month beginning July 1, which adds another volume layer that Q3 will begin to quantify.

Lilly’s P/E ratio of roughly 43 times trailing earnings sits below its five-year median of 53 times. For a company growing revenue near 50% annually with a pipeline this deep, that compression looks like an opening rather than a warning sign.

What Could Go Wrong

The Foundayo price war is not settled. If Novo defends its oral market share aggressively, Lilly’s oral sales could continue missing. Retatrutide’s side-effect profile at higher doses is a monitoring item. And at about $1.1 trillion in market cap, even a modest guidance miss on October 29 tends to translate into a sharp single-day drop. The stock fell 2.5% on October 23 alone without a specific catalyst.

The Bottom Line

Lilly’s injectable franchise, Mounjaro and Zepbound, is growing fast enough to carry the stock even if Foundayo disappoints again in Q3. The EU endorsement expands the addressable market materially. Retatrutide, if approved, resets the competitive landscape. At current prices, investors are paying a below-median multiple for a company with arguably the best-positioned drug pipeline in global pharma. Wednesday’s report won’t answer every question about oral GLP-1 pricing. What it will do is show whether the underlying business, injection volumes, guidance, and new pipeline data, justifies the altitude.
