Disc Medicine Surged 70%. Is IRON Still Worth Buying Today?

APOLLO delivered. A $350 million equity raise the same night changes the math for new buyers.

Monday was a career day for Disc Medicine. The company announced October 19, 2026 that bitopertin met every primary and secondary endpoint in the Phase 3 APOLLO trial in erythropoietic protoporphyria, including X-linked protoporphyria. The stock had already climbed sharply before the opening bell, and it kept moving. By the close, IRON had gained about 39% on the session. Then, after the market shut, the company announced a $350 million equity offering. The question is whether any of that remaining gain belongs to a new buyer.

Start with what APOLLO actually showed. At month six, bitopertin reduced whole-blood metal-free PPIX by 41.4% versus placebo and increased average monthly total time in light without pain by 12.2 hours during month six. The company also reported a statistically significant reduction in the incidence rate of phototoxic reactions versus placebo over the six-month treatment period, but it did not quantify that reduction as 71% in its topline release. Those are not soft numbers. Exploratory quality-of-life endpoints were also statistically significant. The drug works, and the topline release left little ambiguity about that.

Context matters here. In February 2026, the FDA issued a Complete Response Letter for the bitopertin NDA. Company disclosures at the time said the main objection related to the sufficiency of PPIX as a surrogate biomarker, with the FDA indicating it needed to see results from the ongoing Phase 3 APOLLO study before making a decision. APOLLO was, in other words, a mandatory remediation trial, not a bonus data point. Investors likely viewed the all-endpoint win as a major regulatory and commercial positive precisely because the program had previously received that Complete Response Letter. Disc has said it plans to resubmit the NDA to the FDA by the end of 2026, with an FDA decision anticipated by mid-2027. The data clears the path. The approval timeline is real.

Now for the part that complicates a buy today. On the same evening of October 19, 2026, Disc Medicine announced an underwritten public offering targeting $350 million through common stock and pre-funded warrants. The company also intends to grant underwriters a 30-day option to purchase up to an additional $52.5 million of shares at the public offering price. Jefferies, Leerink Partners, Morgan Stanley, and Cantor are serving as joint book-running managers. The offering signals that Disc intends to commercialize bitopertin itself rather than partnering, which is a legitimate strategic choice. It also means the company is selling shares into a roughly 70% move, which is standard biotech playbook but creates concrete dilution for open-market buyers.

Insider activity is a separate question. This draft cited significant net selling totaling $106.5 million over the past 12 months, but that figure was not supported by a verifiable company filing or a reliable consolidated insider summary. Without a source that holds up, it is better not to treat that number as actionable.

The honest case for buying here rests on two things: the FDA decision in mid-2027 and the size of the prize. EPP is rare but severely underserved. Bitopertin is an oral, once-daily pill, dosed at 60 mg in APOLLO. Patients receiving bitopertin experienced an additional 12.2 hours of average monthly total time in light without pain during the sixth month of treatment. For a patient population that organizes daily life around sunlight avoidance, that is a meaningful clinical change, and it gives commercial teams a real story to tell.

The case against buying today is simpler. The APOLLO data was binary, and the binary resolved. Whoever owned the stock before Monday’s open captured the full move. What remains is a regulatory bet on a mid-2027 decision, held through dilutive offering mechanics that can pressure shares near-term as the deal prices and closes.

Disc Medicine is a legitimate company with a confirmed Phase 3 win and a credible path to approval. But the easiest entry in bitopertin was before October 19, 2026. For investors considering a position now, waiting for the offering to price and absorb into the float is the more disciplined approach. Chasing a big gap on trial day, with a $350 million raise hanging over the stock, is a trade that history rarely rewards.
